Inputs

- the results of compliance evaluation,
- what came out of worker participation and consultation,
- changes in risks and opportunities,
- the adequacy of resources,
- the views of interested parties.
Outputs
The output of the meeting is not minutes but decisions: a conclusion on the system's continuing suitability, improvement opportunities, changes to be made, resources to be allocated, and any updated objectives. A review with no resource line identifies problems and leaves the fix undecided.
Frequency
“At planned intervals” — in practice annually is common, but covering every input properly in one meeting is hard. In larger organisations, splitting the subjects across shorter, more frequent meetings works better than one long meeting a year.
Who must be there
As the name says: top management. A meeting the safety specialist prepares and presents to themselves, with no decision maker present, is not a review; because its output cannot be a decision, it does not meet the standard's requirement either.
Link with the audit
Audit findings turn into resources here. Findings left open all year are usually open for lack of resource, and this is the only place that decision gets made.
