The condition for the system
A near-miss system works on one condition: that the reporter comes to no harm. A worker questioned, scolded or marked after reporting will not report again, and the system falls silent within months.
It must be easy
Reporting should take no more than half a minute and be possible in field conditions. A long form means few reports.
Feedback is essential
Telling the reporter what happened is what raises report numbers most. If a report shows no consequence, the worker treats the system as idle.
What the number means
A low number of reports is not good news; it usually shows the system is not working. Rising reports mean rising trust, not rising danger.
