Safety of the scene first
What must happen before notification is making the scene safe and completing the response. Only then does recording and notification follow.
Notified to whom
The required notification is made to the body the legislation designates, within its period. Internal and official notification are different things: the internal record opens at once, the official notification is made within its own deadline.
What is written
Who, when, where, doing what work; the event itself without interpretation; witnesses; the first response and what followed. Cause is not written at this stage — cause is the output of the investigation, not of the notification.
Preserving the scene
Changing the scene before the investigation starts largely removes the chance of finding the root. Unless safety requires otherwise, the scene is preserved and photographs and measurements are taken.
