Guide

How and within what period is a workplace accident notified?

Notification is the first link in the accident record; once its deadline is missed, a later investigation does not make up for it.

Safety of the scene first

What must happen before notification is making the scene safe and completing the response. Only then does recording and notification follow.

Notified to whom

The required notification is made to the body the legislation designates, within its period. Internal and official notification are different things: the internal record opens at once, the official notification is made within its own deadline.

What is written

Who, when, where, doing what work; the event itself without interpretation; witnesses; the first response and what followed. Cause is not written at this stage — cause is the output of the investigation, not of the notification.

Preserving the scene

Changing the scene before the investigation starts largely removes the chance of finding the root. Unless safety requires otherwise, the scene is preserved and photographs and measurements are taken.

In Optifora

The incident module opens the record, reminds of deadlines, gathers witnesses and evidence in the same file, and ties the investigation to the record. A notification nearing its deadline is surfaced in the list.

  1. Complete the responseTreat the injured and make the scene safe.
  2. Open the recordOpen the internal record at once: who, when, where, doing what.
  3. Preserve the sceneAs far as safety allows, preserve it; take photographs and measurements.
  4. Hear the witnessesTake witness accounts immediately after the event and separately.
  5. Make the official notificationCarry out the required notification within its period.
  6. Start the investigationOpen the root cause investigation against the record.

Manage this in Optifora

Optifora is not a single program but a compliance platform assembled from modules. The catalogue states which module is ready today and which is on the roadmap.
